On May 12, 2015, set numbers 7246-7251 were being towed to Coney Island Yard by a 77 locomotive on the (Q) line when a electrical malfunction caused the 1st car (7246) in the set to activate the emergency brakes. The first car screeched to a stop, and the second car (7247) smashed into 7246, throwing the first car onto the northbound track, derailing cars 7246 - 7248. This happened near Times Square, and service on the Broadway lines were halted while MTA crews tried to get the train back on the tracks. The lead car #7246 was completely derailed, as it broke through the 3rd rail and smashed onto the other track, but it suffered a bit of frame damage and underbody damage. 7247 was derailed at the back, but suffered coupler and bumper damage in the front. 7248 was derailed off the tracks in the front as well, but did not suffer any significant damage. After 3 hours of work, the MTA managed to place the derailed train back onto the tracks and it was sent to Coney Island Yard for inspection. The damage to the lead car was significant at the underbody, while the damage to 7247 and 7248 was not serious, the MTA had to take all three cars permanently out of service. The following day, Mikeawaski Rail Co. agreed to supply 3 new cars to the displaced set since the contract was still active. Mikeawaski Rail Co. will not charge the MTA for the three new cars being built. The rest of the set 7249-7251 was sent to 207 Street Yard and are being stored until the three new cars are built. The MTA decided that the cars involved in the accident were to be made into work cars as of May 13, 2015. The lead car #7246 and #7247 were decided to be converted to pump cars, while #7248 will be added to a existing work train set as a rider car. As of May 14, 2015, work is already going underway to convert #7246 and #7247 to pump trains.

The lead car and second car #7246 and #7247 were renamed #P7246 and #P7247 and are both being stored at 207 Street Yard undergoing conversion.

The third car #7248 is being renamed to #W7248 and is being converted to a work train/garbage train/Rider Car.

It is unknown if the last "A" car of the displaced set will be converted to a Work or Pump Train, as the MTA is still deciding. The first two pump cars might be paired up with the other R110A pump cars, but that's all a myth for now.
